- Basic application form
(includes basic biographical information for the student and his/her family) - $35 application fee and photograph of student (must be submitted with the basic application form)
- School records from all schools previously attended (within past three years)
- Principal/Counselor recommendation (optional – there is no form enclosed; this can be obtained by requesting that a principal, director or counselor write a letter)
- Parent questionnaire
- Applicant questionnaire (students entering grades 5-8 only)
- Teacher recommendation(s) (three recommendations for students entering grades 5-8)
- Evaluation and/or testing, depending upon grade level (there is a testing fee)
The Office of Admission will schedule:
- Student visit
- Parent interview
